  • Contract length of 12 months
    • Job Requirements
    • Completion of primary COVID-19 vaccination series or up-to-date COVID vaccine
    • Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or proof of immunity
    • Documentation of TB skin test screening or negative QuantiFERON TB Test within 3 months of start date
    • Physician Statement signed by MD/DO/NP/PA within 365 days of start date
    • Documentation of Hepatitis B vaccination or proof of immunity (preferred)
    • Vaccine required during flu season (Sept 15th - April 30th)
    • Documentation of a negative chest x-ray if positive on TB test
      • Schedule Information
      • Monday - Friday

        • Additional Details
        • A completed primary COVID-19 vaccination series is required
        • No exemptions or waivers for COVID-19 vaccination will be considered
        • Strongly preferred documentation of Hepatitis B vaccination
