• ARRT, BCLS certifications required by time of submission.
  • Job Details
  • Rotating weekends and holidays included.
  • Call only on holidays.
  • Please have all certifications (ARRT, BCLS) by time of submission.
    • Job Requirements
    • Primary Source Verification of professional license and/or certification required.
    • Proof of COVID Vaccine series OR Bivalent booster required.
    • Proof of 2 immunizations or positive titer for MMR required.
    • Fit Test for N95 mask is required.
    • Hepatitis B Vaccination Series or Positive Titer required.
    • Influenza Vaccine documentation required from September-May.
    • If history of positive PPD/TB blood test, a Negative Chest X-Ray and Annual TB Questionnaire are required.
    • Copy Valid Drivers License required.
    • 2 Step PPD OR IGRA (Quantiferon/Tspot) required.
    • Proof of 2 immunizations or Positive Titer for Varicella required.
    • Proof of Tdap Immunization within 10 years required.
      • Additional Details
      • Proof of COVID Vaccine series OR Bivalent booster required; Medical exemption reviewed and approved prior to start.
      • Hepatitis B Vaccination Series, Proof of Immunization, Fit Test, and other medical documents must include identifiable information.
