Job Description:

A company is looking for a Care Coordinator - Clinical Operations to provide outreach services and support in healthcare settings.

Key Responsibilities

Conduct outreach services via phone and other channels to complete Social Determinants of Health (SDOH) assessments
Make referrals to in-house pharmacy teams based on assessment results for various care needs
Document all inquiries and maintain professionalism while interacting with customers

Required Qualifications

High School Diploma or equivalent
1+ years of experience in accessing community resources
Bi-lingual proficiency in Spanish and English
Knowledge of managed care or medical experience preferred
Ability to navigate multiple technology systems
