  • Job Details
  • 1-year assignment.
  • BSN and 2-years of experience with Neurology.
  • This position entails following a patient that has had a stroke or seizure 90 days post discharge.
  • Follow up with the patient if they go to a SNF/LTAC make specialist or PCP follow up appointments medication management home healthcare and will assist with getting the patient to a clinic if there are noted concerns.
  • Homecare or case management experience preferred.
  • M-F 40 HPW.
